Embedding Forms & Polls

A profile on United Pet Club can show any of its live forms — a poll, a this-or-that, a quiz, an event RSVP or a longer form — on another website. The embed is a small, self-contained card: the question, the results its owner shows everyone, and an Answer on United Pet Club button. Answering always happens on United Pet Club, signed in, so the answer is tied to a member and counted in the owner’s analytics under the EMBED channel.

To put a whole form on your page, so visitors fill it in there (contact, quote and booking requests, applications), see Embed forms on your website.

The form’s owner copies it from the Dashboard: Paperwork → Forms → Responses → the form → Insights → Embed. It looks like this:

<iframe src="https://www.unitedpetclub.com/embed/f/{deploymentId}?locale=en-US"
title="Best walk time?" width="100%" height="360"
style="border:0;max-width:560px" loading="lazy"></iframe>
PartMeaning
{deploymentId}The form deployment’s id (a GUID). An unknown or retired id renders This form isn’t available. with HTTP 404.
localeOptional, any of the platform’s locales (fr-FR, es-MX, …); the card’s labels follow it. Default en-US.
height360 px fits a poll with up to four choices; a longer form card needs about 200 px. The card does not resize the frame.
  • It is served from the www origin as a standalone HTML document: no site header or footer, no ads, no cookies set, no analytics, and no JavaScript. Its stylesheet is inline and follows the visitor’s light or dark preference.
  • Its Content-Security-Policy is default-src 'none'; style-src 'unsafe-inline'; img-src https: data:; base-uri 'none'; form-action 'none'; frame-ancestors https: — any https page may frame it, and nothing in it can run.
  • Counts show only as the owner chose for everyone (results shown always, or once the form closed); a visitor’s own answer is never shown in the embed.
  • The button opens /{locale}/f/{deploymentId}?ch=EMBED in a new tab. There the visitor signs in (or creates a free account) and answers.
  • Responses are cached for 60 seconds.
  • Only https pages can frame the card (and http://localhost for testing).
  • The card is the owner’s content: don’t restyle or overlay it in a way that hides the United Pet Club attribution line.
  • There is no API key and nothing to register.
